All charters need to be entered virtually at  Wisconsin 4-H Annual Charter Application or handed in to the Extension Office by November 1.  Remember you will need to submit SMART Goals for your club/committee.  You will also need to list out all potential activities for the year.  Please check all boxes that might be a possibility.  

In addition to these forms you will also need to submit to the Extension Office

• current club / group by-laws
• club meeting calendar (dates and locations of monthly meetings)
• Previous years minutes and attendance (if your club/committee hasn’t been putting them in the google folder)
